Teraligen (Alimemazin)

Active substance: Alimemazine tartrate 

Pharmacological action:

Antipsychotic (neuroleptic). Has antihistaminic, antispasmodic, serotonin-blocking, moderate alpha-adrenoceptor blocking, antiemetic, hypnotic, sedative and antitussive effect. 

Antipsychotic effect due to blockade of dopamine D 2 receptors and the mesolimbic mesocortical system. Has low antipsychotic activity, so in acute psychotic states ineffective. 

Sedative effect due to blockade of adrenergic receptors of the reticular formation of the brain. 

Antiemetic effect due to blockade of dopamine D 2-receptor trigger zone of the vomiting center. 

Hypothermic effect due to blockade of dopamine receptors of the hypothalamus. 

Due to the good tolerability used in pediatric, adolescent and geriatric practice.

Dosage

The drug is taken orally, distributing daily dose for 3–4 hours. 

Onset of action - after 15–20 min, duration - 6–8 hours 

Adults to achieve the hypnotic effect of 5–10 mg/day to achieve anxiolytic effect by 60–80 mg/day. In psychotic states - 200–400 mg/day. 

Children with 7 years (depending on age and body weight) to achieve a sedative effect at 2.5–5 mg/day as the symptomatic treatment of allergic reactions 5–20 mg/day to achieve an anxiolytic effect at 20–40 mg/day, with psychotic conditions may increase the daily dose to 60 mg/day. 

Drug Interactions:

Alimemazin enhances the effects of opioid analgesics, hypnotics, anxiolytics (tranquilizers) and antipsychotic (neuroleptic) drugs, and drugs for general anesthesia, m-cholinergic antagonists and antihypertensive drugs. Should adjust the dose alimemazin while the use of the aforementioned drugs.

At simultaneous application with alimemazin derivatives amphetamine, m-cholinomimetics, ephedrine, guanethidine, levodopa, dopamine weakened action of the latter.

At simultaneous application with alimemazin ethanol and drugs that suppress the central nervous system, CNS depression is observed.

With simultaneous application alimemazin with antiepileptic drugs and barbiturates reduced seizure threshold (requires correction dose).

With simultaneous application alimemazin with beta-blockers increased the concentration of the plasma (possibly pronounced decrease in blood pressure, arrhythmias).

At simultaneous application with bromocriptine alimemazin waning effect of the latter and increases the concentration of serum prolactin.

With simultaneous use of tricyclic antidepressants alimemazin and anticholinergic drugs are amplified m-anticholinergic activity alimemazin.

With simultaneous use of MAO inhibitors alimemazin and phenothiazine derivatives increased risk of hypertension and extrapyramidal disorders (concurrent use is not recommended).

With simultaneous application alimemazin with drugs which suppress bone marrow blood, increases the risk of myelosuppression.

At simultaneous application with alimemazin hepatotoxic drugs increase the expression of the drug hepatotoxicity.
Pregnancy and lactation:

Do not use this drug during pregnancy and breastfeeding.
Side effects:

CNS and peripheral nervous system: drowsiness, lethargy, fatigue (mainly occur in the first days of admission and rarely require discontinuation of therapy), paradoxical reaction (anxiety, agitation, nightmares, irritability), rarely - confusion, extrapyramidal disorder (hypokinesia, akathisia, tremor); quickening apnea, increased seizure activity (in children).

From the senses: blurred vision (accommodation paresis), noise or ringing in the ears.

Cardio-vascular system: dizziness, decreased blood pressure, tachycardia.

From the digestive system: dry mouth, gastrointestinal atony, constipation, loss of appetite.

The respiratory system: dry nose, throat, increasing the viscosity of bronchial secretions.

Urinary system: atony bladder, urinary retention.

Other: allergic reactions, suppression of bone marrow hematopoiesis, increased sweating, muscle relaxation, photosensitivity.

Usually the drug is well tolerated by patients. Side effects are rare and expressed slightly.
